- Silver coloured metal medal. On the obverse central to the design is a relief depiction of a masonic temple. Encircling this is a border with engraved text reading 'DOUGLAS LODGE #12 GENOA, NV / 125TH ANNIVERSARY / 1868-1993'. On the reverse central to the design are relief depictions of various masonic emblems. These include a relief letter 'G' enclosed within a set square and compass, a level, a plumb rule, a radiated All-seeing eye, and a rule overlaid with a trowel and gavel crossed in saltire.
