Top bar: Trapezoid shaped bar in gold plated silver with engraved design (slipped down to below first ribbon device). Ribbon: Ribbed silk in dark red. First ribbon device: Gold plated silver bar, matt with text in relief reading '14 JUNE 1897'. Second ribbon device: Gold plated silver banner infilled with red enamel and with text reading '1901-2-3-4'. The banner is held in the beaks of two birds also in gold plated silver, and with a red enamel panel between them with text bearing 'M.E.Z.' Bottom bar: Trapezoid shaped bar in gold plated silver with engraved design. Hanging device: Eastern crown on a triangle, both in gold plated silver. They are framed within a gold plated silver roundel, matt and with text in relief reading 'POLISH / NATIONAL / CHAPTER', with a sunburst visble beneath. Pin: Missing.
